Responsibilities
Provide overall leadership and operational oversight for the Case Management department in an acute care hospital setting
Direct and evaluate case management models, staffing plans, skill mix, FTEs, onboarding, and staff competencies
Ensure effective utilization management, discharge planning, and care coordination processes
Monitor length of stay (LOS), throughput, productivity, and quality outcomes
Lead performance improvement initiatives using data analysis and operational metrics
Ensure compliance with hospital policies, CMS regulations, Joint Commission (TJC) standards, and payer requirements
Collaborate with executive leadership including CFO, CMO, CNO, Legal, Ethics & Compliance, and ancillary service leaders
Promote interdisciplinary collaboration to ensure continuity of care and optimal clinical resource utilization
Manage daily operations while driving long-term strategic goals for the department
Qualification
Required
Bachelor's degree (required)
Active RN, LCSW, or LMSW license (required)
Minimum 3+ years of acute care hospital case management experience
Minimum 2+ years of leadership or management experience in hospital case management
Strong knowledge of utilization review, discharge planning, care coordination, and regulatory compliance
Excellent communication, leadership, and presentation skills
Preferred
Master's degree in Nursing (MSN), Healthcare Administration (MHA), or Business Administration (MBA)
Experience working in large health systems or multi-department hospital environments
Proven ability to lead teams, manage change, and drive performance improvement
