Responsibilities

Responsible for verifying new medication orders, checking prepared medications, overseeing dispensing from the pharmacy, and completing clinical patient review for safety and accuracy
Provides safe, efficient, high quality, patient-centered, and evidence-based medication services to all patients
In cooperation with other pharmacists and pharmacy technicians, acts as a resource and provides answers to drug information questions for patients and family members, pharmacists, nurses, physicians, advanced practice providers, medical students, undergraduate and graduate pharmacy students, and other associated hospital personnel
Screens for drug-drug, drug-herb/dietary supplement, drug-allergy, drug-food, drug-disease state, and drug-laboratory interactions
Obtains and maintains clinical privileges, if applicable
Using clinical privileges, adjusts medications based on route of medication administration access, monitors and adjusts medications based on renal, hepatic, antithrombotic indications, and hematologic parameters, orders and adjusts laboratory tests related to monitoring medication therapy, and modifies and adjusts Total Parenteral Nutrition therapy and associated fluids and electrolytes in conjunction with a dietician, if applicable to role
Participates and provides support in the Emergency Response effort, including Code Blues and Stroke Alerts
Engages in appropriate hospital and/or departmental committees relevant to practice area
