CARE COORDINATOR (Pivot /OP )
Crossroads Turning Points is dedicated to helping individuals navigate the challenges of substance misuse and mental health. They are seeking a compassionate Care Coordinator to coordinate care for patients and support them on their journey to recovery.

Responsibilities
Responsible for the development of care planning and patient needs assessments
Facilitates life skills groups in accordance with the CTP policies and procedures
Acts as liaison, establishing networks and collaboration with hospitals, community health centers, and state and human service agencies about patient services and needs
Document admissions, care management needs, discharge, and discharge planning
Administer case management assessments to identify patient needs and ensure the Addiction Severity Index (ASI) is completed at intake
Organize a schedule of patient bed dates, transitioning levels of care, and consistent communication with the Medicaid Placement Coordinators
Assists treatment team with patient intake, transfer, and discharge assessments
Collaborating with the facility treatment team (medical, psychiatric, and SUD) to identify patients’ needs
From available resources, assist patients in meeting their basic needs (i.e., clothing, medical, judicial, and religious)
Qualification
Required
A bachelor's degree in human services or a related field
Obtain the Certificate of Addiction Technician (CAT) certification within 18 months of the hire date
Obtain case management certification within 90 days of hire
Minimum of two (2) years of experience working in the substance use and mental health field
Must have a good understanding of substance misuse, mental health, and treatment delivery systems
Advanced computer skills required
CPR and First-Aid certification (the agency pays for both initial training and the renewal courses)
Must be able to take vital signs, recognize possible medical problems, and refer to the medical staff when necessary
Valid driver's license and good driving record
Able to communicate effectively in a multidimensional treatment program, including medical, psychiatric, and substance misuse-related issues
