Responsibilities
Collaborate closely with nurse practitioners (NPs) and interdisciplinary care teams to coordinate member care
Conduct routine monthly health assessments and visits as delegated by NPs
Review resident status and support progress toward individual care goals
Attend meetings on behalf of NPs and communicate updates effectively with the interdisciplinary care team
Perform care coordination activities within skilled nursing facilities to ensure resident needs are met
Assist with clinical and quality initiatives, including addressing gaps in care
Engage regularly with residents, families, facility staff, and care teams
Help facilitate Advanced Care Planning and perform resident rounds
Primarily assigned to one facility per day with occasional travel to other locations for urgent visits
Qualification
Required
Nursing degree
active registered nurse (RN) license
minimum 1+ years RN experience
Preferred
Associate or Bachelor of Science in Nursing (ASN or BSN)
Valid North Carolina RN license
Basic Life Support (BLS) certification
Minimum of one year experience in Long Term Care, Care Coordination, or Care Management
