Bilingual Case Manager (Clackamas Health Center)
Clackamas County is dedicated to serving the public and enriching the community. They are seeking a Bilingual Case Manager to provide comprehensive care coordination and case management services to individuals facing mental health and addiction challenges, working collaboratively within a multidisciplinary team.

Responsibilities
Coordinate care for clients with mental health and/or addiction issues, including referrals to resources, monitoring treatment, and assisting with discharge planning
Work with doctors, nurses, and insurance companies to keep clients stable and help them transition between services
Interview clients, families, and staff to gather information for treatment plans and determine appropriate services
Maintain accurate case records and documentation according to agency policies and regulations
Provide counseling and support to clients and families, including guidance on daily living skills, job readiness, and medication management
Collaborate with healthcare providers and social services to secure necessary services for clients
Advocate for clients by working with families, providers, and agencies to resolve issues and respond to emergencies
Qualification
Required
Qualified Mental Health Associate (QMHA) or (QMHA- R) as established by the Mental Health and Addiction Counseling Board of Oregon (MHACBO) as mandated by Oregon Administrative Rule at the time of hire
Fluency in Spanish/English, both oral and written (bilingual skills to be validated/tested at time of interview and/or selection)
Must possess and maintain a Basic Life Support (BLS) certificate
Experience providing case management services to clients diagnosed with severe and persistent mental illness (SPMI), and/or substance use disorders (SUD)
Preferred
Experience working in a primary care or behavioral health care setting
Experience providing services and/or support to vulnerable clients
Experience working with Evidence Based Treatment Practices
