Responsibilities
Maintaining a caseload of vulnerable consumers
Exercising initiative and independent judgment when discharging assignments
Gathering information from consumers, family members, and others necessary for identifying problems
Conducting home visits
Creating care plans
Setting up and ordering services with providers
Actively participating in meetings
Providing guidance, education, and support to consumers and caregivers
Qualification
Required
Six months as an Aging Care Manager 1 or a County Caseworker 1; or
Successful completion of the County Social Casework Intern program; or
Six months of professional human services or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or
An equivalent combination of experience and training
You must meet the PA residency requirement
You must be able to perform essential job functions
