Perform comprehensive assessments, guide care plans, and work directly with physicians to ensure each patient’s needs are met
Keep your notes sharp—EMR documentation within 24 hours!
Travel to patient homes (yes, this includes navigating stairs and weather surprises) to deliver exceptional care
Educate, comfort, and empower families during their hospice journey
Collaborate with our interdisciplinary team to achieve positive outcomes
Qualification
Required
Graduate of an accredited nursing program
RN License (active and in good standing)
1+ years of nursing experience (hospice experience required)
A valid driver's license, insured vehicle, and a sense of adventure for the road ahead
Preferred
Bilingual (Spanish/English)? Amazing!
