Housing Liaison

The City of Norwalk is currently seeking a Housing Liaison with experience in social services, shelter placement, and housing advocacy. The role involves assessing clients' housing needs, creating individualized plans for housing stability, and providing case management and outreach services to connect clients with housing resources.

Responsibilities

Conducts daily street outreach in encampments, riverbeds, and any locations where people experiencing homelessness temporary resides to develop relationships, build rapport, conduct interviews, and assess the needs for housing. Collects through phone, home, community, and office contacts and assesses client information; coordinates plans to address housing needs with social service and community agencies
Provides street-based case management and navigation services to assist clients and their families obtain housing stability
Assists clients with housing applications, completes supportive and subsidized housing paperwork, surveys rental market for affordable housing, and advocates for clients with prospective landlords
Establishes professional networks with landlords, renter associations, property management companies, and other local housing providers to identify potential rental properties and provide permanent housing options for clients
Develops and monitors individual housing case plans with each client to mitigate barriers to housing stability; works collaboratively with employment case managers to create comprehensive case plans that may include the need for specialized social services
Maintains thorough case documentation for each assigned client; updates and maintains client records; gathers and compiles data from written and multi-agency specific computer software for reporting purposes; develops summary records and prepares reports
Responds in a timely manner to request for street outreach services and sends follow-up information to the requestor and supervisor
Expands expertise in the field of service through participation in educational opportunities; attends multi-disciplinary HOPE team and task force meetings to develop collaborative efforts in service delivery
Develops and maintains strong ties to homelessness prevention service providers. Works alongside law enforcement, non-profit organizations, homeless service providers, the City entities service providers to ensure transition from street living to interim or permanent housing. Collaborates with community substance abuse and mental health providers to ensure seamless referral services
Maintains confidentiality of client files per HIPAA and all applicable guidelines
Establishes and maintains cooperative working relationships. Provides high quality customer service for clients and collaborating with partner agencies. Emergency callouts and the ability to work evenings and weekends may be required
Conducts street outreach at the Metro Green line on a weekly basis or as assigned
Homeless Management Information System (HMIS) knowledgeable
Performs other duties as assigned

Required

Self-motivated, willing to learn, able to meet deadlines, and can exercise sound independent judgment
Prior housing navigation or housing case management experience is desirable
The ability to speak Spanish is required
Any combination of education and experience that has provided the knowledge, skills, and abilities necessary for satisfactory job performance
A combination of 30 semester or 45 quarter units in Social Work, Sociology, Psychology, Child Development, Gerontology, Vocational Counseling, or a closely related field or (3) years of case management work experience in public, non-profit, or private social services agency
Possession or ability to obtain an appropriate California Driver's License and a satisfactory driving record at the time of appointment

Benefits

Retirement: Membership in the Public Employee's Retirement System (PERS) is as follows: Employees hired after January 1, 2013 who are new to Cal-PERS, or are returning members with a break in service greater than six months, will be enrolled into the 2% @ 62 retirement plan formula and will be required to pay up to 50% of the "normal costs" currently 8.5%). Returning members with a break in service less than six months may be enrolled into the 2% @ 55 retirement plan formula and will be required to pay 7% of their reportable compensation (employee contribution), if eligible. Furthermore as a result of PEPRA, CalPERS will take into account prior service at a reciprocal retirement system (or system that has similar agreements with CalPERS, such as CalSTRS) when determining your benefit formula when you enter membership with a CalPERS employer.
Employee Assistance Program: An Employee Assistance Program (EAP) is available to provide help to employees and members of their families who are experiencing personal problems. For more information contact the Department of Human Resources.
Paid Holidays: The City currently observes ten holidays plus two floating holidays.
Vacation Leave: Eighty hours per year after completion of one year; 120 hours per year after completion of 5 years; 160 hours per year after completion of ten years or more.
Sick Leave: Credit for sick leave with pay shall accrue at the rate of (8) hours for each calendar month of service. A 3,000 hour Hourly Unit employee who is hired to a full-time position, shall have the right to use any previously earned sick leave while serving their probationary period in the new promotional classification. However, the probationary period will be extended by the amount of time used.
Medical Plan: Each employee has a choice of insurance plans offered through the California Public Employees' Retirement System (CalPERS). The City currently pays the monthly premium cost up to $1,600.
Dental Insurance: Covered through Delta Dental Plan. The City pays 100% of the premium for employee plus dependents.
Vision Insurance: Covered through Vision Services Program (VSP). The City pays 100% of the premium for employee plus dependents.
Group Life Insurance Plan: Life insurance is provided at no cost to the employee. The City provides $5,000 term insurance to employees with less than 5 years of service. Effective the first month following completion of 5 years continuous service, employees are covered with insurance equal to one and one-half times their annual salary.
Voluntary Insurance Plans: Optional Supplemental Life, Short Term Disability, Hospitalization, Accident, and Cancer insurance policies are available at the employee's cost.
Long Term Disability Insurance: The City pays 100% of the insurance plan.
Deferred Compensation: Option plan available at employee's cost.
