Geriatric MD/DO - Rutland
Community Health is a nationally recognized primary health care network providing multi-disciplinary care to the communities of central Vermont. They are seeking a physician to lead a new geriatric service line, which includes direct patient care and consultative roles, along with program design and clinical oversight.

Responsibilities
Lead a novel geriatric service line
Provide direct patient care and consultative/educational care for the local provider community
Program design
Clinical care
Oversight of clinical operations
Establish and nurture relationships with internal and external stakeholders
Serve a consultative and supportive role to the independent SNF service line
Qualification
Required
Graduate from accredited medical school and FM/IM residency
Board certification in Internal Medicine or Family Medicine
Must be eligible for licensing by the State of Vermont
Related experience working with geriatric population
Effective communication sensibility
Preferred
Geriatric fellowship and subsequent Board certification recommended (not required)
