RN - Local Office of Community Care

The Marion, IL VA Health Care System is seeking two full-time Registered Nurses (RNs) within the Local Office of Community Care (LOCC). This role is responsible for establishing and monitoring seamless care for Veterans, coordinating clinical components of community care services, and ensuring integrated care with VA services.

Responsibilities

Establish, promote and monitor seamless care for Veterans
Act as a resource to the medical center staff, contracted and networking facilities
Execute position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex client care within the VHA Community Care Program
Organize and coordinate the clinical components for the unauthorized/authorized care of Community Care services in response to individual healthcare needs along the illness and care continuum and in multiple settings
Coordinate patient care in the community and meet the Medical Center's utilization goals in a manner that successfully integrates the community care with past and future VA Care
Collaborate with all clinical services across the Medical Center and with the administrative staff within the LOCC to ensure coordinated, timely, cost-efficient care is provided to the Veteran
Act on behalf of the Medical Center and with other VISN facilities to determine availability of clinical services, ensure proper transfer of patient care, participate in collaborative case management planning, utilization review, discharge planning, and other related case management activities including the review of outside medical services based on established guidelines and medical necessity
Utilize multiple computer program systems efficiently to coordinate veteran care

Required

U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy

Must pass pre-employment physical examination
Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP)
You may be required to serve a probationary period
Complete all application requirements detailed in the 'Required Documents' section of this announcement
English Language Proficiency. In accordance with 38 U.S.C. 7403(f), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English
Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE); OR Individuals attending a master's level bridge program in nursing who have completed coursework equivalent to a bachelor's level degree in Nursing may have opportunity to become registered as a nurse with a state licensing board prior to completion of the bridge program
Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia

Preferred

Three (3) years of experience in an acute care setting and/or other clinical care setting
Current community care and case management/utilization review experience
Proficiency with Microsoft Office products (outlook, excel, and access)
BSN preferred
