Social Services Program Coordinator II

NEOGOV is seeking a Social Services Program Coordinator II for the Ellisville State School, a state-operated program that provides services to individuals with intellectual and developmental disabilities. Responsibilities

Development and implementation of a person-centered Plan of Services and Supports(PSS)
Coordinate and monitor all services the person receives through the IDD Community Support Program to ensure services are adequate, appropriate, and meet the person’s needs
Monitoring follow-up activities include contact with the person receiving services and/or his/her legal representative, service providers or other entities at least monthly
Conduct quarterly face to face assessments of each person both in the home and service setting
Document all contacts and adhere to requirements in the IDD Community Support Program Targeted Case Management Manual
Must have use of personal vehicle to conduct on site visits

Typically requires a Bachelor's Degree and 2-4 years of experience
Typically requires a Bachelor's Degree in a human services-related field or at least a Bachelor's Degree in a non-related field and at least 2-4 years of relevant work experience
Relevant experience is defined as full-time experience working directly with people with intellectual and/or developmental disabilities, or other types of disabilities, or mental illness
Must have use of personal vehicle to conduct on site visits
