Responsibilities
Conduct comprehensive clinical reviews of patient pharmacotherapy, including utilization, counseling, compliance, interaction screening, side effect profiling, and administration support
Document pharmacotherapy reviews clearly in patient charts for nursing follow-up
Counsel patients on new drug therapies in alignment with clinical management models
Complete REMS training and adhere to associated SOPs
Coordinate patient care with referring clinicians, addressing social, psychological, and physical needs requiring customized services
Serve as liaison among patients, caregivers, nurses, hospice teams, and prescribing clinicians
Provide subcutaneous injection education to patients and caregivers; coordinate nursing services as needed
Provide on-call coverage as directed by the Pharmacist-in-Charge
Process prescriptions accurately and timely
Verify discontinuation of patient services and deactivate charts, prescriptions, and profiles as appropriate
Implement clinical management models for specific products and disease states
Serve as primary intake for new patient referrals via telephone
Deliver 'outrageous customer service' in accordance with internal standards
Foster strong relationships with patients and clinicians
Adhere strictly to company policies related to government-funded payers, including Medicaid and Medicare
Dispense medications per physician orders and in compliance with all applicable laws
Ensure compliance with pharmacy operations policies and controlled substance regulations
Verify accuracy of prescription order entry and final dispensed products
Support pharmacy technicians with claims adjudication and payment accuracy
Monitor hazardous waste material management
Supervise aseptic technique and oversee pharmacy technicians
Utilize patient education materials effectively
Intake and report adverse events and product quality complaints per regulatory requirements
Maintain and exceed continuing education (CE) requirements
Support professional development of assigned staff
Commit to ongoing learning within the pharmacy field
Attend professional conferences and share insights with staff
Qualification
Required
Must live within 1 hour driving distance of Fort Worth, TX
Minimum 3 years of counseling experience
Comfortable being on the phone 7–8 hours per day
Licensed and in good standing in the state of Texas
3–5+ years of post-graduate experience in pharmacy operations, clinical oversight, and patient care
Graduate of an accredited school of pharmacy
Current, unrestricted pharmacist license in good standing
Licensed in North Carolina or Texas to practice pharmacy
Ability to apply for additional state licensure as needed
Excellent oral and written communication skills
Strong computer and word processing proficiency
Working knowledge of sterile technique and quality control standards
Preferred
BCOP certified or willing to become BCOP certified when eligible
