Responsibilities

Lead comprehensive care coordination that integrates physical health, behavioral health, and social determinants
Reduce hospitalizations and emergency interventions through proactive, evidence-based treatment planning
Improve client stability, housing retention, and community integration through coordinated support
Collaborate with hospitals, courts, law enforcement, primary care providers, and community agencies
Create seamless care transitions that keep clients connected to the services they need
Strengthen EBCAP's reputation as a leader in community-based mental health care
Mentor and coach a multidisciplinary team through ongoing supervision and competency development
Implement evidence-informed practices that reflect the latest in ACT model fidelity
Foster a culture of continuous improvement, accountability, and compassionate care delivery
Maintain Joint Commission standards and state regulatory compliance
Monitor performance metrics that demonstrate program effectiveness
Lead quality improvement initiatives that enhance service delivery and client outcomes

Qualification

Master's Degree in human services3-5 years clinical experienceManagerial experienceCo-occurring disordersMedication-Assisted Treatment (MAT)BHDDH regulationsEmpathyData-driven decision makingCollaborative relationshipsTrauma-informed care

Required

Master's Degree in human services, Bachelor's Degree in human services with a LCDP or RN license
3-5 years post-graduate clinical experience in rehabilitative community-based mental health
1-2 years of managerial and supervisory experience
Experience serving individuals with Severe and Persistent Mental Illness
Knowledge of co-occurring disorders and Medication-Assisted Treatment (MAT)
Familiarity with BHDDH regulations

Preferred

Background in IHH, ACT, CSP, or similar intensive community-based models
Knowledge of ACT fidelity standards and state regulations
