Responsibilities
Provide Supportive Services. Help individuals with severe mental illness and/or intellectual/developmental disabilities; some with co-occurring substance use disorders
Conduct Assessments. Evaluate needs and conditions to better understand each person’s situation
Create Individual Service Plans. Develop personalized plans using a person-centered approach to meet each person’s unique needs
Monitor and Evaluate. Regularly check progress and performance to ensure that service plans are effective
Maintain Confidential Records. Keep all information secure and private, adhering to confidentiality standards
Qualification
Required
Degree and licensure in Social Work, Psychology, Counseling preferred, however, other Human Service or Nursing degrees will be considered
Valid driver's license required
Ability to work in Houghton County, Michigan, and travel to surrounding counties as needed
Able to pass a criminal background check
Preferred
Experience working with those with intellectual/developmental disabilities and/or mental illness is preferred
