Responsibilities

Support daily intake and care coordination workflows
Review referrals and patient records for completeness, accuracy, and timeliness
Ensure documentation meets internal standards and deadlines
Identify missing, incorrect, or delayed items and route for follow-up
Coordinate with intake, clinical, billing, and administrative teams
Monitor assigned workflows to ensure procedures are followed consistently
Perform routine quality checks on documentation and operational tasks
Identify recurring issues or gaps and escalate appropriately
Maintain trackers, reports, and internal documentation
Support implementation of updated workflows and SOPs
Ensure records and workflows align with internal compliance standards
Assist with internal audits and documentation reviews
Support preparation for accreditation and survey activities (e.g., JCAHO, DPH)
Maintain organized, audit-ready records

Required

Bachelor's Degree
3 years of experience in healthcare coordination, intake, or operations support
Strong attention to detail and ability to manage multiple tasks
Process-oriented mindset with ability to follow and validate procedures
At least 1 year of experience coordinating with remote teams
Strong organizational and problem-solving skills
Clear written and verbal communication skills
Comfortable navigating and working within EMR systems
Demonstrated working knowledge of Microsoft Office (Word, Excel, Outlook)
Ability to work onsite with minimal supervision
Availability to work one weekend per month, as needed

Preferred

Home Health experience
Exposure to compliance, audits, or regulated healthcare environments
Clinical background (preferred, not required)
