(1519) IDD Crisis Intake and Admissions Coordinator
Bluebonnet Trails Community Services is dedicated to improving the health and independence of individuals with intellectual and developmental disabilities. The IDD Crisis Intake and Admissions Coordinator serves as the point of entry for IDD programs, responsible for enrolling eligible individuals into various Medicaid/GR programs and providing advocacy and service linkages.

Responsibilities
Enrolling eligible individuals into Medicaid/GR programs
Providing services to children and adults with Autism, Developmental or Intellectual disabilities
Completing eligibility screenings for various funding streams from multiple referring agencies
Facilitating service linkages and monitoring for effectiveness
Providing crisis intervention/prevention supports and advocacy services
Working with families and various agency personnel to accomplish objectives
Participating in interagency planning and CRCG meetings to assist in bridging gaps in services
Conducting responsibilities in accordance with HHSC Principles and Medicaid requirements
Qualification
Required
A bachelor's degree or higher with a major in social, behavioral, or human service field including, but not limited to psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, or criminal justice
Must have a valid Texas driver's license and maintain approved driver's status throughout employment
Preferred
At least one year case management or service coordination experience working with individuals diagnosed with Intellectual or Developmental Disability is preferred
Knowledge of appropriate community resources and experience with IDD programs and Medicaid Waiver programs is ideal
Familiarity with the SmartCare program
Master's Degree
