Phaxis is seeking a dedicated Advanced Practice Provider (NP or PA) to join their outpatient Internal Medicine / Family Medicine team. This role focuses on building long-term patient relationships while providing comprehensive primary care services.

Provide preventive care, annual physicals, and wellness visits

Diagnose and manage acute and chronic conditions

Manage medication therapies and care plans

Order and interpret labs and diagnostic testing

Educate patients on disease management and preventive health

Coordinate referrals and specialty care

Maintain accurate documentation in the EMR

Collaborate with physicians and care team members

Qualification
