Responsibilities
Perform initial and ongoing hospice eligibility assessment and consultation with Hospice MD in collaboration with Stanford team and in accordance with CMS guidelines
Assume primary responsibility for a patient/family using the nursing process to assess, plan, implement and evaluate patient/family needs, goals, and interventions
Perform evaluation and coordination of social work and spiritual support services needed by patient/family
Coordinate the implementation of the plan of care and ensure care coordination with the inpatient team
Maintain regular communication with the Stanford Hospital team and other team members involved in the Inpatient program
Qualification
Required
One year experience as a professional nurse in the past three years or; Bachelor's degree in Nursing from an accredited School of Nursing
Current license as a registered professional nurse in the State of California
Preferred
Experience in acute care/ home health and/or hospice setting preferred
Public Health Nursing certification highly desirable
HPNA certification preferred
