Responsibilities
Stabilize care delivery through early risk detection and appropriate escalation
Create continuity across visits and handoffs so care feels coherent rather than fragmented
Translate provider intent into reliable execution across follow-ups and next steps
Reduce operational friction by coordinating with MAs, case management, and operations
Strengthen patient understanding and agency through clear, respectful communication
Maintain defensible documentation that supports quality and regulatory standards
Qualification
Required
Active CT Registered Nurse license in good standing
1–3 years of clinical experience within RN scope
Strong clinical judgment and prioritization skills
Effective communication with patients, providers, and staff
Comfort operating in a community health and ambulatory environment
