Responsibilities

Conducts assessments, engages with clients, and explains eligibility. Conduct phone and in-person eligibility assessments for individuals seeking assistance through Medicaid eviction and homelessness prevention programs
Engage with clients using a trauma-informed approach, demonstrating empathy, sensitivity, and respect for their unique circumstances and experiences
Utilize a Diversity, Equity, and Inclusion (DEI) lens in all interactions and decision-making processes, ensuring equitable access to services and addressing any barriers or disparities marginalized communities face
Collect and accurately document relevant client information in the designated database system, in accordance with confidentiality and data protection protocols
Collaborate with program coordinators and caseworkers to review assessment findings and determine eligibility for program enrollment
Provide clients with information and resources on available support services, community resources, and additional assistance options
Participate in ongoing training and professional development activities to enhance knowledge of trauma-informed care practices, DEI principles, and program eligibility criteria
Maintain compliance with program guidelines, regulations, and reporting requirements, ensuring accurate and timely submission of documentation and reports

Required

A minimum of two (2) years of related experience working with vulnerable populations, particularly individuals experiencing homelessness or housing instability, that would provide the required knowledge and skills to perform the responsibilities of this position
Demonstrated commitment to diversity, equity, and inclusion, with an understanding of systemic inequities and their impact on marginalized communities
Proficiency in data collection and documentation, with experience using database systems or case management software
Proficiency in Microsoft Office Suite (including Outlook, Teams, etc.)
Knowledge of trauma-informed care principles and ability to apply them effectively in client interactions
Ability to work independently and collaboratively in a fast-paced, high-pressure environment, prioritizing tasks and managing time effectively
Strong communication and interpersonal skills, with the ability to establish rapport and build trust with clients from diverse backgrounds
Sensitivity to confidentiality and privacy concerns, with a commitment to upholding ethical standards and maintaining client confidentiality at all times

Preferred

Experience working in a Medicaid eligibility, social services assessment, or other service screening role
Familiarity with eviction prevention and/or homelessness prevention programs
Proficiency in a second language, particularly Spanish, to better serve diverse community members
Training or certification in trauma-informed care practices
Knowledge of state and federal regulations related to Medicaid and housing assistance programs
