Jordan Valley Community Health Center is a mission-driven organization dedicated to improving the health of individuals and families in underserved communities. The PACE Physician will be responsible for providing primary care services to elderly patients in a Program of All-Inclusive Care for the Elderly (PACE) setting, ensuring high-quality care and individualized care plans.

Responsibilities
Provide primary care services to elderly patients in a PACE setting
Conduct comprehensive assessments of patients' medical, social, and functional status
Develop and implement individualized care plans for each patient
Coordinate care with interdisciplinary team members, including nurses, social workers, and therapists
Monitor patients' health status and adjust care plans as needed
Provide education and counseling to patients and their families
Participate in quality improvement activities and initiative
Qualification
Required
Medical degree from an accredited institution
Board certification in Internal Medicine or Family Medicine
Active medical license in Missouri
Excellent communication and interpersonal skills
Ability to work effectively in a team-based care model
Commitment to providing high-quality, patient-centered care
Preferred
Experience working with elderly patients preferred
