Senior Case Manager

ServiceNet is a leading Human Services Agency seeking compassionate and dedicated individuals to join their team. The Senior Case Manager will coordinate supports for families with school-age children and young adults with intellectual disabilities and/or autism, performing various duties including family support and team collaboration.

Support families with school-age children, teenagers, and young adults with intellectual disabilities and/or autism
Provide family supports and recruit in-home service providers
Collaborate with larger team members and co-supervise a small team of Case Managers

Required

Compassionate, dedicated, and enthusiastic individuals
Ability to create and coordinate supports within the family home
Highly collaborative with other team members
Experience supporting families with school-age children, teenagers, and young adults with intellectual disabilities and/or autism
Ability to provide family supports
Experience recruiting in-home service providers
Ability to collaborate with larger team members
Experience co-supervising a small team of Case Managers
Ability to provide services to families
Flexibility to meet case load needs and adjust hours as needed
Ability to report to office when needed
Ability to conduct in-person outreach supports throughout Hampden, Hampshire, Franklin, and Berkshire Counties
