Responsibilities
Conduct in-home RN assessments throughout Richmond, VA and surrounding areas
Monitor and document changes in client condition
Develop and oversee individualized plans of care
Supervise and evaluate caregiver performance
Participate in quality assurance activities, including care plan reviews and client conferences
Educate clients and families on disease processes, treatments, medications, and nutrition
Conduct in-home visits with prospective clients and families
Serve as a clinical representative of the agency during prospect visits
Build and manage a caseload that includes enrolled clients and assigned referrals
Transition new clients into ongoing RN oversight
Complete accurate, timely documentation using digital forms and agency software
Manage cases within defined productivity standards
Qualification
Required
Active RN license in the state of Virginia
Valid driver's license and reliable vehicle
Minimum of 5 years of nursing experience (home health, senior care, or related field preferred)
Strong technology skills; ability to quickly learn new software systems
Confident communicator comfortable meeting with prospective clients and families
Excellent organizational, time-management, and multitasking skills
Ability to maintain confidentiality
Comfortable working in homes with pets (cats and dogs)
Preferred
Experience with Google Calendar, Adobe, electronic documentation, and agency platforms
