Responsibilities

Dispense prescription medications and provide patient counseling
Review and interpret prescriptions for accuracy and clinical appropriateness
Monitor medication therapy for potential drug interactions or adverse effects
Collaborate with physicians, nurses, and clinical staff
Maintain complete and accurate patient medication records
Ensure compliance with federal, state, and pharmacy regulations
Support medication therapy management and patient education initiatives

Qualification

Required

PharmD required
Active pharmacist license (or ability to obtain)
Strong knowledge of pharmacology, anatomy, and physiology
Familiarity with aseptic technique and medication administration
Strong communication and interpersonal skills
High attention to detail and accuracy
Ability to work effectively in a multidisciplinary healthcare team
Experience with pharmacy software systems
Understanding of insurance billing and reimbursement processes

Preferred

Experience in home infusion, acute care, critical care, or utilization review preferred
Pharmacy residency experience is a plus
