Responsibilities
Process and manage incoming referrals for CGMs and insulin pumps
Review and verify the completeness of patient documentation, including prescriptions, clinical notes, insurance information, and physician orders
Follow up with referring providers and clinics to obtain missing documentation as needed
Act as a primary point of contact for referring offices, prescribers, and internal teams regarding diabetes DME referrals
Coordinate with the insurance verification and authorization teams to ensure timely approvals
Communicate with patients regarding referral status or to gather additional information when required
Ensure all referrals and patient records meet payer-specific and regulatory documentation guidelines
Maintain accurate records in the electronic health record (EHR) and DME software systems
Support audits and quality checks by maintaining organized and compliant documentation
Identify and suggest improvements to referral workflows for efficiency and patient satisfaction
Participate in team huddles, training, and process improvement initiatives
Qualification
Required
High school diploma or equivalent required
2+ years of experience in healthcare intake, medical referrals, or DME services
Strong understanding of insurance requirements for DME referrals
Excellent communication and organizational skills
Proficient in EHR and DME management software systems
Ability to manage multiple tasks in a fast-paced environment with high attention to detail
Preferred
Associate's degree or healthcare-related certification preferred
Prior experience with diabetes-related DME (CGMs and pumps) highly preferred
