Clinical Pharmacy Specialist - HBPC & Ambulatory Care
The Iowa City VA Health Care System is seeking a dedicated Clinical Pharmacy Specialist to improve the lives of Veterans. This hybrid role involves delivering high-quality patient care through comprehensive medication management in Home Based Primary Care and Ambulatory Care settings.

Responsibilities
Conduct comprehensive medication management for Veterans enrolled in the HBPC program, ensuring the safe, effective, and evidence-based use of medications
Collaborate with an interdisciplinary team to develop individualized care plans that optimize therapeutic outcomes and enhance the quality of life for each patient
Perform patient assessments, monitor therapeutic responses, and adjust medication regimens accordingly
Provide education and counseling to patients and caregivers on medication therapy, disease state management, and lifestyle modifications
Conduct home visits as necessary to evaluate patient adherence, identify potential medication-related issues, and provide direct patient care
Deliver advanced clinical pharmacy services in Ambulatory Care clinics, focusing on chronic disease management, anticoagulation therapy, and other specialized therapeutic areas
Provide direct patient care through face-to-face or telehealth appointments, ensuring the delivery of high-quality, patient-centered care
Review and evaluate medication orders for appropriateness and effectiveness, making recommendations for therapy adjustments as needed
Review nonformulary medication consults to ensure appropriate, cost-effective medication therapy
Collaborate with primary care providers and other healthcare professionals to achieve optimal therapeutic outcomes and promote medication safety
Engage in continuous quality improvement initiatives to enhance clinical practices and patient outcomes

English Language Proficiency: Pharmacists appointed to direct patient care positions must be proficient in spoken and written English
Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree
Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia
Experience: You must have 1 year of experience at or equivalent to the next lower grade level (GS-12)
Knowledge, Skills, and Abilities: You must demonstrate the following knowledge, skills, and abilities: Ability to communicate orally and in writing to persuade and influence clinical and management decisions
Expert understanding of regulatory and quality standards for their program area
Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise
Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy
Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters
Preferred
Expertise in chronic disease management, therapeutic drug monitoring, medication therapy management, and patient counseling
Residency training or board certification in pharmacotherapy or a related specialty is preferred
