HEALTH CARE SERVICES TRAINEE II

Responsibilities
Under the direction of the LPT, LPN, RN, and within the scope of training provided: administers and documents residential, therapy, medical and behavior related objectives as identified in the IPP of persons served
Under the direction of the LPT, LPN, RN, and within the scope of training provided: performs or assists and trains residents on all activities of daily living (ADL). This includes but is not limited to: meal preparation; personal hygiene; social skills; recreation activities; housekeeping and laundry; safety and transportation, all with a training approach
Under the direction of the LPT, RN, and within the scope of training provided: De-escalates agitating behaviors by utilizing skills learned to mitigate the situation safely. Performs physical intervention and redirection techniques when needed, utilizing the least restrictive measures available
Within the scope of QMAP training provided and the delegation of a nurse, administers medications and treatments to the individuals served in accordance with the Physician’s orders. Administers physician ordered medications and medical treatments in proper doses at appropriate times to each individual. Records a wide range of information including: medication given (includes time and amount); residents’ responses to medications and treatments; intake, output, seizures, menses; and all information relevant to all aspects of the clients’ medical status

Qualification
Required
Passed a CDPHE & WRRC approved QMAP course
State of Colorado residency at the time of application
Must be at least 18 years of age
Must possess a valid, non-restricted Colorado Driver's License or a non-restricted U.S. Driver's License for positions within 30 miles of the CO state border
Must travel to remote work sites and must utilize a state vehicle, if available
Pre-employment Physical/Test: Must demonstrate the ability to lift 50 lbs, bend, squat, etc., and perform physical interventions proficiently
Influenza and other vaccines as required by the State of CO
A Tuberculosis Test (TB) will be required upon hire
Must be available to work any shift, any day, and/or any area, to meet agency needs
Motor Vehicle Check, if the employee will drive a state-owned vehicle
CBI name check and fingerprint check, ICON Colorado court database, Medicare fraud database, Reference Checks, Professional License verification, Drug Screen, PREA, Trails check, CAPS
