Client Care Advocate / Sales & Marketing
Leading Home Care is the leading Home and Community-Based Services provider for Medicaid waiver services in Louisiana, seeking a Client Care Advocate to enhance client intake through community partnerships and education. The role involves building relationships with referral sources, guiding clients through service eligibility, and ensuring compliance with regulations.

Responsibilities
Visit and build relationships with referral sources including hospitals, doctor’s offices, hospices, rural health clinics, and other community agencies as applicable to drive client referrals
Serve as a resource to partnering agencies to ensure smooth coordination of care for potential clients of LHCL
Provide community education on services provided by LHCL by hosting informational meetings or in-services with other organizations
Host and support social marketing functions sponsored by referral partners to engage in networking and promotion of LHCL
Seek new referral sources in the community to stimulate client leads
Attend workshops, seminars, and conferences that provide information or resources that may assist LHCL with providing quality support services for clients
Present a professional image on behalf of LHCL and be accessible and accommodating to referral sources, prospective clients, and families
Be readily available to complete initial meetings with clients, which may be scheduled with little notice at the convenience of the client or family
Build initial rapport and become a trusted advisor to clients and families
Guide potential clients through eligibility requirements for services that they may have access to
Clearly communicate with all roles involved in initiating client services (Client Care, Staffing, Billing). Serve as liaison to monitor the process and ensure clients are receiving appropriate communication and timely follow-up
Use software system to track leads and document progress towards starting services
Maintain all aspects of confidentiality according to HIPAA, LDH & LHCL
Follow policies, procedures, and guidelines established by LHCL, Medicaid program manuals, and LDH-HCBS licensing standards
Model the LHCL Mission, Values, & Leadership Behaviors
Additional tasks related to Marketing or Client Advocacy as needed
Qualification
Required
Minimum 2 years of clinical, hospice, HCBS, or home health marketing required
Must possess strong people-skills, ability to network effectively, and be an effective communicator
Basic computer skills including Windows & Microsoft Office
Capacity to operate office equipment
Basic addition/subtraction and general analytical skills
Valid driver's license, auto insurance, vehicle inspection, and reliable personal transportation; frequent travel is required within Region 6
Must be able to read, write, and follow instructions in English
Must satisfactorily pass all required and applicable State & Federal eligibility verifications including background, driver's license, and other required screenings
Physical requirements include stooping, kneeling, reaching, standing, walking, and climbing stairs. Must be able to occasionally lift up to 25lbs (carrying files, marketing supplies, boxes, etc.)
The ability to interact on a professional level with LHCL clients and their families, DSWs and representatives of other community-based organizations
Preferred
Degree in Marketing, Healthcare Administration, Communication, or Business preferred
