The role involves entering ICD-9 and CPT coding for procedures, abstracting data from patient health information, and ensuring the completion of charts in collaboration with physicians.

Responsibilities
Responsible for entering all ICD-9 and CPT coding for both in-patient and out-patient procedures
Responsible for the abstraction of data from the patients health information
Responsible for entering the abstracted data into the information system
Selects charts and completes part of the review form as functions for Medical Records Committee
Interacts with physicians to assure completion of charts
Performs other duties as assigned
Qualification
Required
High School diploma or GED equivalent with courses in typing and computer basics
Must be able to type a minimum of 50 words per minute
Must possess the clerical perception necessary to proofread and to avoid errors in transposition
Previous health information experience
Must possess physical ability to endure prolonged sitting; moderate walking; lifting and carrying
May require exerting up to 20 pounds of force occasionally; and/or up to 10 pounds of force frequently; and/or a negligible amount of force constantly to move objects
Physical Demand requirements are in excess of those for sedentary work
