Responsibilities
Perform comprehensive assessment of hospice patients
Collaborate with primary care providers to ensure implementation of individualized care plans
Continuously reassess cost-efficient, appropriate levels of care
Coordinate care transitions with physicians, social workers, and discharge planners
Engage specialty resources for optimal resolution of patient needs
Identify opportunities for health promotion and illness prevention
Act as patient advocate by protecting privacy and confidentiality
Qualification
Required
Education: Associate or Bachelor's degree in Nursing (or related field)
Attendance: Commitment to mandatory attendance during the first 90 days
Case Management certification
Strong ability to analyze complex medical information and make sound clinical decisions
OASIS and HCHB experience
Multi-state RN license
HH EXPERIENCE - IVS, WOUND VACS, OASIS, OSTOMIES, FOLEYS, CHEST TUBES
Current TB test
CPR/BLS certification
Wound care competency
Valid insurance and ability to travel locally
