Pharmacist - Pediatrics ED & PICU - Houston, TX (Evening Shift)
GDR is seeking a Residency-Trained Integrated Pediatrics Pharmacist to join their team in Houston, Texas, supporting the Emergency Department and Pediatric Intensive Care Unit. The role focuses on delivering safe, high-quality medication management and pharmacy support for pediatric patients while overseeing pharmacy technical staff and providing drug-related education.

Responsibilities
Fill and label prescriptions; provide drug-related information and education to patients, families, hospital employees, and medical staff
Dispense controlled substances and maintain accurate inventory records
Supervise pharmacy technical staff and mentor pharmacists, pharmacy students, or residents
Participate in Medication Use Evaluation (MUE) programs and Process Improvement initiatives
Maintain and review medication profiles for drug-related problems; provide consultations on drug research
Prepare and schedule IV solutions and additives; ensure proper compounding practices
Report medication errors or adverse drug interactions; ensure compliance with all safety standards
Provide clinical interventions, including:
Medication profile review
IV-to-oral conversions
Renal dosing adjustments
Antimicrobial streamlining
Patient counseling and medication history collection
Therapeutic drug monitoring (e.g., vancomycin, aminoglycosides)
Participation in multidisciplinary rounds
Document clinical interventions accurately in the patient record
Serve as preceptor and mentor, supporting professional growth and departmental goals
Ensure adherence to hospital policies, procedures, and standards, including safety, productivity, and quality of service
Perform additional duties as assigned to support department operations
Qualification
Required
Graduate of an accredited School of Pharmacy
Licensed as a Registered Pharmacist in Texas
Sterile Product Certification (in-house or accredited program) if compounding sterile products
Minimum 2 years as a hospital pharmacist, with knowledge of unit-dose medication and IV admixture processes
Strong clinical judgment and decision-making skills
Ability to supervise, mentor, and train pharmacy staff
Knowledge of pediatric pharmacy practices, emergency medicine, and critical care
Commitment to patient safety, compassion, and personalized care
Effective communication and collaboration within a multidisciplinary team
Preferred
PALS certification
