They are seeking an Admission / Discharge Coordinator to support admissions and discharges in their crisis therapeutic homes and serve as a liaison with community stakeholders.

Responsibilities
Provide admission and discharge support to Youth Crisis Therapeutic Home, Adult Crisis Therapeutic Home, and Adult Transitional Home
Participate in the Admission Advisory Committee for the Adult Transitional Home
Serve as a liaison with community stakeholders
Responsible for all discharge planning including support team meetings, community appointments, and overall discharge communication
Maintain individual support plans and participate in the residential on-call rotation
Qualification
Required
Bachelor's degree in a human service or related field
Qualified Mental Health Professional, or eligible to obtain QMHP-T within 30 days of start
At least 1 year of experience with both developmental disabilities and/or mental health populations
Maintain a valid driver's license and auto insurance
Preferred
Crisis prevention and intervention experience preferred
