Responsibilities
Provide general habilitation and companionship care as indicated in the Individual Support Plan (ISP)
Report directly to Client Care Managers (CCM’s) and communicate regularly as needed
Be available during scheduled shifts and comply with all JEVS Care at Home policies
Conduct announced and unannounced home visits two times annually during a scheduled shift in the client’s residence
Maintain the integrity of JEVS Care at Home and comply with the Caregiver Manual
Communicate any Electronic Visit Verification (EVV) compliance issues with the Client Care Manager (CCM)
Qualification
Required
Must be 18 years of age or older
Must have a clean driving record, reliable vehicle with updated insurance, registration, and inspection
Must meet minimum qualifications for each of the following background checks: Criminal, Child Abuse, OIG, SAMS and Medicheck
Must be a U.S. Citizen or authorized to work in the U.S
Must have a reliable vehicle that is insured, with updated registration and inspection
Arrive to assignments with reliability and punctuality
Must satisfactorily complete JEVS Home Care written exam at 80% passing score on post orientation exam and orientation program
Ability to effectively communicate with Client Care Manager
Completion of Caregiver Orientation
Must be able to function in a home setting with minimal direct supervision
Must be able to follow verbal and written instructions and document services provided
Must be genuinely concerned about helping people and have high moral standards of honesty and integrity
The employee is frequently required to stand; walk and sit
The employee must occasionally lift and/or move up to 50 pounds, and have the ability to assist with client ambulation and transfers to/from bed to/from bath and toilet
While performing the duties of this job, the employee is regularly required to use hands; reach with hands and arms
Preferred
High school diploma or GED preferred
