Responsibilities
Conduct comprehensive initial in-person visits to assess patients and their home environment, develop the initial care plan, and set treatment goals for the Virtual Care @ Home program
Proactively identify eligible patients from inpatient, ER, or clinic settings. Perform clinical assessments to determine suitability and coordinate with physicians for referral approval
Serve as the primary educator for patients and families, explaining the virtual care program, technology, self-care techniques, and disease management. Advocate for patients by identifying social or environmental barriers to success
Work in close partnership with an Admitting Specialist, Social Workers, Paramedics, and the Virtual Care physician team to coordinate patient consent, equipment setup, and a timely discharge from the facility into the home-based program
Actively seek opportunities to refine and improve the admissions and enrollment process to enhance patient satisfaction, outcomes, and health system efficiency
Qualification
Required
Registered Nurse (RN)
Current Georgia Registered Nurse (RN) license or Multi-State Compact RN license
Associate Degree in Nursing (ADN), Diploma in Nursing, or Bachelor of Science in Nursing (BSN) required
BLS (Basic Life Support) required upon hire
ACLS (Advanced Cardiac Life Support) required within 180 days of hire
Minimum 3 years of acute care nursing experience in ICU or medical-surgical units required
Exceptional patient education, communication, and interpersonal skills
Strong clinical assessment and care coordination abilities
Must be self-directed, comfortable with technology and telehealth concepts, and adept at working within a collaborative, interdisciplinary team
A valid driver's license and reliable transportation are required for in-home visits
Preferred
Master's in Nursing (MSN)
Minimum 5 years of experience in Home Health or Hospice
