Responsibilities
Responsible for the effective operation of the adult day health center
Coordination and communication with other departments
Supervision and training of day center staff
Facilitating and scheduling the development of Plans of Care
Compliance with the policies, procedures, and regulations affecting Community LIFE
Assess, plan care for and provide service to frail elders per their assigned responsibilities and Community LIFE standards
Qualification
Required
Bachelor's Degree in Nursing, Business Administration, or a health or human services field
A Minimum of five years of health care experience
Three years of management experience, preferably in a geriatric care setting
A minimum of one year's experience in working with the frail or elderly population
Knowledge of the physical, mental, and social needs of the elderly
Valid PA Driver's License
Highly adept at team facilitation and problem/conflict resolution
Knowledge of needs of elderly population in various disciplines
Effective written and oral communication skills
Strong organizational and planning skills; ability to manage multiple priorities
Working knowledge of utilization review, quality assurance, and managed health care concepts
Working knowledge of local health care and geriatric service networks
Preferred
Master's degree highly preferred
