Service and Support Coordinator (Health Pro V) - Grand Junction Regional Center
The Colorado Department of Revenue is seeking a Service and Support Coordinator for the Grand Junction Regional Center, which serves individuals with intellectual and developmental disabilities. This role involves leading and overseeing services, coordinating teams, and ensuring compliance with regulations to support meaningful life outcomes for individuals in residential settings.

Responsibilities
Coordinate and monitor individualized support plans for people with developmental disabilities
Ensure services meet federal, state, and agency regulations and protect program funding
Lead interdisciplinary team meetings with medical, therapy, and support staff
Track progress toward individual goals and adjust plans based on data and observations
Oversee daily service delivery to ensure active treatment occurs throughout the day
Develop and monitor programs that build daily living and adaptive skills
Supervise and support direct care staff, including scheduling and performance management
Provide training and coaching to staff on therapeutic strategies and care techniques
Respond to behavioral challenges using approved intervention strategies
Serve as an on-call duty officer as assigned for after-hours support
Make placement and service recommendations based on individual needs and safety
Ensure proper documentation, audits, and quality assurance reviews are completed
Advocate for individuals’ rights and personal outcomes
Drive continuous improvement in residential and day programs
Perform administrative and supervisory duties as assigned
Qualification
Required
Eight (8) years of relevant experience in an occupation related to the work assigned to this position
A combination of related education and/or relevant experience in an occupation related to the work assigned to this position equal to eight (8) years
Full background check: CDHS employees (all direct contact with vulnerable persons): CBI name check and fingerprint check, ICON Colorado court database, Medicare fraud database, Reference Checks, Professional License verification (licensure requirements), Drug Screen, PREA (Division of Youth Services), Trails check (direct contact with children), CAPS (direct contact with adults – Mental Health Institutes, Regional Centers, Veterans Community Living Centers) and reference check required
This position requires State of Colorado residency at the time of application (unless otherwise identified in the posting), and residency within the state throughout the duration of employment in this position
Motor Vehicle Check, if the employee will drive a state-owned vehicle, either in an on-going capacity while performing their normal day-to-day job duties OR on occasion for training/meetings/client visits/etc
Must possess a valid, non-restricted Colorado Driver's License or an non-restricted U.S. Driver's License for positions within 30 miles of the CO state border
Out-of-State Driver's License holders must obtain a valid, non-restricted Colorado Driver's License within 30 days of employment start date
Travel – Drive state car, van or bussette to various group homes, programming, admin & in community
TB test is required, requires Influenza and other vaccines as required by state and facility; must be 18 years or older
Pre-employment Physical/Tests – Exert up to 50 lbs. of force occasionally, and/or up to 20 lbs. of force frequently, and/or up to 10 lbs. of force constantly to move objects
Preferred
Preferred candidate will have at least year of supervisory experience
Experience working in Home and Community-Based Services Waiver licensed facilities or group homes
Demonstrated knowledge of the Performance Management Plan (PMP) process
Demonstrated ability to communicate effectively and professionally
Demonstrated skills prioritizing and organizing work
Demonstrated ability to interpret rules and regulations
Advanced proficiency with Google applications and general computer use
Proficiency with electronic health records
