Responsibilities
Provide annual physicals and comprehensive preventive care
Manage chronic and acute conditions with an emphasis on whole-person care
Deliver gender-affirming primary care, including hormone therapy management and pre/post-operative support
Offer same-day or next-day appointments with extended visit times
Support mental health and wellness through referrals and coordination
Maintain continuity through telemedicine, secure messaging, and occasional after hours calls
Navigate specialist referrals and hospital coordination as needed
Experience with EMR (Athena or Epic preferred)
Qualification
Required
Board-certified in Internal Medicine or Family Medicine
Experienced in adult outpatient medicine, ideally with 5+ years in private practice or primary care
Culturally competent and affirming in LGBTQ+ care, including experience in transitional/gender-affirming medicine
Committed to equitable, inclusive, and patient-centered healthcare
Skilled in building lasting relationships across diverse communities
A clear communicator with excellent bedside manner
Comfortable working in a low-volume, high-touch concierge model (average 8 patients daily)
Interested in long-term practice growth and leadership
Preferred
Experience with EMR (Athena or Epic preferred)
