Responsibilities
Perform comprehensive assessments of high-risk members in the field
Collaborate with primary care providers to develop and implement individualized, comprehensive care plans
Identify, address, and continuously reassess cost-effective and appropriate levels of care
Coordinate care transitions and engage specialty resources as needed
Arrange non-medical support services (such as housing and transportation) to support treatment compliance
Maintain detailed clinical, functional, and fiscal documentation
Identify opportunities for health promotion and illness prevention
Prevent adverse patient outcomes when possible and intervene promptly when prevention is not achievable
Act as a patient advocate while maintaining privacy and confidentiality
Qualification
Required
Associate's or Bachelor's Degree in Nursing or related field
Current, unrestricted Registered Nurse (RN) license
Case Management certification
Strong ability to analyze complex medical information and make sound clinical decisions
Effective communication and collaboration skills
Ability to work independently in a field-based, metrics-driven environment
