Responsibilities
Perform comprehensive assessments of high-risk members
Collaborate with primary care providers to implement individualized care plans
Identify and reassess appropriate, cost-effective levels of care
Coordinate care transitions across the continuum of care
Arrange non-medical support services such as housing and transportation
Partner with physicians, social workers, discharge planners, and other care team members
Engage specialty resources to support optimal patient outcomes
Maintain detailed documentation of clinical, functional, and financial outcomes
Identify opportunities for health promotion and illness prevention
Act as a patient advocate while ensuring privacy and confidentiality
Qualification
Required
Education: Associate's or Bachelor's Degree in Nursing or related field
Attendance: Commitment to mandatory attendance during the first 90 days
Active, unrestricted Registered Nurse (RN) license in the state of practice
Strong clinical judgment and ability to analyze complex medical information
Preferred
Case Management certification preferred
