Responsibilities
Perform comprehensive assessments of high-risk members
Collaborate with primary care providers to develop and implement individualized care plans
Identify, coordinate, and reassess cost-effective and appropriate levels of care
Arrange non-medical support services (e.g., housing, transportation) to promote treatment compliance
Coordinate care transitions with physicians, social workers, discharge planners, and other care team members
Engage specialty and community resources as needed to achieve optimal outcomes
Maintain detailed documentation of clinical, functional, and financial outcomes
Identify opportunities for health promotion and illness prevention
Prevent adverse patient outcomes when possible and intervene promptly when needed
Act as a patient advocate while maintaining privacy and confidentiality standards
Qualification
Required
Associate's or Bachelor's Degree in Nursing or related field
Active, unrestricted Registered Nurse (RN) license in the state of practice
Strong ability to analyze complex medical information and make sound clinical decisions
Effective communication skills for collaboration with providers, care teams, and members
Ability to manage documentation and outcomes in a metrics-driven environment
Preferred
Case Management certification
