Site Coordinator (Community Living Program, Tucson)

The Site Coordinator is responsible for facilitating Individual Service Plans, providing administrative support, and ensuring compliance with relevant regulations while working closely with Direct Support Professionals to assist individuals in the program.

Responsibilities
Assists in all aspects of assigned residential settings
Under the direction of program leadership and on an ongoing basis, provides training, scheduling, evaluation of work, and constructive performance feedback
Completes staff performance evaluations
Provides staff training and documents training per policies and procedures
Responsible for compliance with Department of Economic Security (DES) and Division of Developmental Disabilities (DDD) Residential Program monitoring rules at all times
Responsible for assuring that client records are maintained to meet guidelines of DES/DDD program monitoring, contract compliance, and agency policies and procedures
Prepares and completes resident ISPs, skill training objectives, teaching plans, and data collection methods
Writes, implements, and reports on Behavior Treatment Plans (BTP) for residents requiring a plan
Conducts regular staff meetings, both individually and as a group, to problem solve, provide information and feedback, and to receive input regarding program and client statuses
Ensures all program operations are conducted in a safe and appropriate manner and reports problems to the Associate Director
Responsible for attendance and accurate reporting at all resident ISP meetings and other pertinent program meetings
Ensures staff provide adequate and timely data collection using the program documentation software
Ensures that all client medical and evaluation needs are met per recommendations of the ISP team, therapists, Primary Care Physicians, etc.; ensures that results of such are accurately documented in individual case files
Completes monthly documentation to DDD Support Coordinator, guardians, and other team members as requested to include, but not limited to, monthly progress reports, medication records, behavior treatment plan data, medical/therapy appointments, and changes in status
Under the direction of the Associate Director, maintains financial accountability of program household and client account monies following identified policy and procedure for all tasks and processes
Maintains a flexible working schedule based upon program need
Participates as an active member of the individual ISP teams, maintaining professional and courteous contacts with parents, guardians, family members, day program representatives, and medical and therapy personnel
Assists in providing direct care at the program sites as directed or required to meet program needs
Responsible for on-call duty in case of emergency; shares on-call responsibility with Lead DSPs
Communicates consistently with staff to include all in-home training, client specific information, changes/updates in client medical conditions and recommendations, program plans, BTPs, and documentation requirements; documents communications, as appropriate
The Site Coordinator will respond to employee issues/concerns and complete assigned employee performance evaluations of staff at sites
Works as a member of the Community Living Program Administration to promote the agency's policies, procedures, and philosophy to co-workers, DDD agents, other providers, and the community at large
Ensures quality indicators are present in the program on a continual basis
Communicates regularly with program leadership with regard to staffing issues, resident concerns, safety issues, timekeeping issues, and established procedural changes
Completes weekly inspection forms for apartments or home
Completes van inspection logs
Creates a monthly activity calendar with residents and staff including recreational and social activities
Performs health and safety inspections
Orders supplies
May drive agency vehicle on company business
Qualification
Required
High School Diploma or GED
One year of related work experience
Excellent verbal and written communication skills with an ability to establish rapport with team members and individuals served
Strong organizational abilities to ensure staffing and schedules are maintained
Knowledge of the DES/DDD Residential Program Monitoring rules
Experience with writing and implementing objectives and skill plans per ISP guidelines
Ability to lift 60 pounds 15% of the time
Other licensure or certifications required by regulatory authority
Must be at least 18 years of age
Valid driver license, proof of insurance, and 39-month motor vehicle report
Ability to obtain and maintain Arizona Level One Fingerprint Clearance Card and FBI National Criminal Records History Report (employer paid)
Pass pre-employment drug screen (includes marijuana, regardless of recreational use laws) (employer paid)
