Remx is seeking a reliable and organized Population Health Care Coordinator to provide outreach and support for patients with Passport/Molina insurance. This role focuses on connecting patients to preventive services, closing care gaps, and improving population health outcomes.

Responsibilities

Schedule patient appointments and follow‐ups
Monitor and complete preventive health measures
Notify providers of diagnoses needing risk adjustment
Use registries and reports to track preventive care needs
Educate patients on recommended screenings
Enter patient data accurately into the EHR
Coordinate interpreter services for patients with LEP
Scan/upload documents into the EHR
Conduct outreach to patients who frequently use the ER and offer primary care appointments
Focus specifically on care coordination for Passport/Molina patients

Qualification

Required

Strong organizational and time‐management skills
Ability to work independently and prioritize deadlines
Excellent communication and customer service skills
