Required
2 - 3 years Acute Care Discharge Planning experience
STRONG two years of Discharge Planning / Care Coordination in an Acute Care Hospital Environment
Must have BLS
Must be actively licensed as a RN in Tennessee
Must have two - three or more years of working with in Case Management / Discharge Planning for an Acute Care hospital
