Responsibilities
Providing trauma informed case management services, including but not limited to symptom monitoring and ongoing assessment of needs, to clients and/or their family system in while incarcerated
Providing consultation and collaboration with community partners and clients’ treatment teams
Documenting program procedures and policies, Treatment Plans, Progress Notes Discharge Plans and assisting clients with applications for benefits/resources
Qualification
Required
Associate's Degree in Human Services/Social Work or related field required
Minimum of one (1) year relevant experience
Ability to obtain QMHS/QBHS within 15 days of hire as provided by employer
Must have an acceptable driving record (4 points or less), reliable transportation, and carry automobile insurance with minimum liability limits of $100,000/$300,000 and property damage of $50,000
Preferred
Bachelor's degree preferred
Experience working with an incarcerated population preferred
