Responsibilities
Support the execution and the ongoing refinement of Population Health programs
Work closely with functional leaders (Product, Finance, Data Science) in the central office to assist with strategy and program design
Work with regional leaders and Care Teams to ensure effective implementation of key Oak Street clinical and business priorities
Lead communication and content development related to key population health metrics, including quality measures, utilization measures, and financial measures
Other duties, as assigned
Qualification
Required
5+ years of applicable experience in consulting / healthcare / population health / working with healthcare providers; or other similar fields
Ability to build the trust and confidence of a wide range of stakeholders
Track record of effectively solving difficult problems
Practical, results-oriented mindset
Ability to manage multiple priorities while maintaining a positive attitude
Ability to develop business cases, ROI analysis, and financial analysis
Someone who connects the dots between teams - a proactive communicator who puts individual projects into broader organizational objectives
Ability to succinctly summarize and synthesize large amounts of information
Expert in MS Office, Google Apps, project management software
