Responsibilities
Connect with North Carolinians who are applying for Medicaid-funded in-home Personal Care Services (PCS) to support their activities of daily living
Assess them in their home
Complete an online report which will guide service eligibility and level of care determinations
Daily travel within Nash, Edgecombe and Halifax Counties is required
Qualification
Required
North Carolina Registered Nurse (RN) license
Experience in home health, hospice or a similar field-based nursing role
Computer proficiency and good keying skills
Good organizational and time management skills – and an ability to meet daily timelines
Residency within Nash, Edgecombe or Halifax County is required
