Responsibilities
Practice exceptional medicine within knownwell
Sees adults for primary care, often with a focus on metabolic health, or adults for metabolic health only. Share a panel with MD/DO + other APCs
Embodies the knownwell mission and values of patient inclusivity in how you deliver care
Coordinates with the broader care team to ensure the patient has the support they need
Documents as necessary to ensure accurate billing, including review of remote patient monitoring data
Communicates with patients in a timely manner through our portal for questions and concerns that have been pre-screened to necessitate a clinician response
See adults ages 16+ for metabolic health and primary care
Ensure panel is open to any individual, regardless of their current health status and comorbidities
Listen to the aspirations of the patient and ensure care is tailored to the patient’s unique health journey. Provide an environment in which patients feel comfortable and free from judgment
Offer exceptional encouragement as patients reach their goals
Coordinate across the collaborative care team
Escalate any concerns to your dedicated site leadership team
Provide clinical guidance to medical assistants and nursing teams as assigned
Stay current with licensure and appropriate re-certifications
Qualification
Required
Master's degree (MSN) and certification by one of the Nurse Practitioner certifying bodies (ANCC or AANPCB) as a family nurse practitioner
Unrestricted DEA licensure and prescribe authority (per state practice regulation)
At least 2 years of clinical experience in an ambulatory care setting, family or internal medicine; experience in chronic condition management and weight management preferred
Active, unrestricted license to practice in Massachusetts, or willingness to obtain
Willingness to obtain additional licensures if needed based upon business needs
Excellent communication and interpersonal skills
Dedication to staying current with the latest developments in primary care and obesity medicine
Belief in inclusive, compassionate care
Belief in shared decision making with a patient
High emotional intelligence and ability to make a patient feel at ease – without judgment and without shame
Willingness and excitement to try new technologies that are both patient and provider oriented
Excitement to work with a collaborative team of physicians, APC's, registered dietitians, medical assistants, health coaches, and other staff
Authorized to work in the United States
Preferred
