Manager of Quality Assurance

Responsibilities

Oversee the full incident management process from report to closure
Ensure all steps of the process meet regulatory timelines and standards
Assign and track investigations for quality and compliance
Conduct weekly investigator meetings and daily QA office huddles
Review investigations and provide detailed feedback and corrective action guidance
Monitor and ensure completion of all incident follow-up requirements
Identify health and safety risks and escalate appropriately
Serve as primary point of contact for OPWDD Incident Management Unit
Monitor IRMA submissions, alerts, and regulatory communication
Maintain professional relationships with OPWDD IMU and the NYS Justice Center
Respond promptly to regulatory inquiries and documentation requests
Ensure accurate classification and filing of reportable incidents and 150s
Maintain up-to-date knowledge of OPWDD 624 regulations
Coordinate, schedule, and facilitate IRC meetings
Prepare agendas, take minutes, and ensure completion of all IRC recommendations
Triage IRC requests and assign investigations appropriately
Facilitate the monthly Informed Consent Committee
Supervise investigative and clerical staff, including coaching and accountability
Ensure high-quality training and professional development for investigators
Provide mentorship and support best practices in investigations
Analyze and trend incident data to identify systemic risks and opportunities
Prepare incident trend reports to support leadership decision-making

Required

Supervisory or leadership experience in quality assurance, compliance, or investigations
Experience providing oversight or advisement in an investigative or regulatory environment
Strong knowledge of OPWDD services and regulatory requirements
Excellent written communication and attention to detail. Needs to be able to have a high degree of accuracy and consistency when reviewing investigative reports
Ability to handle sensitive situations with professionalism and clarity
Proficiency in Microsoft Office, including Access
Experience reviewing, analyzing, and trending incident reports

Preferred

Prior experience supervising investigators within OPWDD or Justice Center reporting frameworks
Familiarity with IRMA and OPWDD incident reporting systems
Experience facilitating committees or multidisciplinary review groups
Prior experience in medical or behavioral health settings
