Responsibilities

Provide flexible nursing support across case management, admissions, and after-hours roles based on organizational needs
Conduct clinical assessments, evaluate patient eligibility, and complete admissions including consents and initiation of hospice services
Manage a hospice caseload when assigned, including symptom management, care coordination, and interdisciplinary collaboration
Respond to urgent patient and family needs during after-hours or on-call shifts, including symptom control, medication coordination, and death pronouncements
Coordinate care with attending physicians, facility staff, and other providers to ensure aligned, goal-concordant hospice care
Provide emotional support, education, and guidance to patients, families, and caregivers during all phases of care
Ensure accurate, timely documentation to support clinical, regulatory, and billing compliance
Maintain current knowledge of hospice eligibility, services, and regulatory requirements
Participate in staff meetings, training, and quality improvement activities as assigned
Perform other duties as assigned

Required

Graduate of an accredited nursing program; Associate's degree or higher preferred
Current RN license in practicing state or a valid multistate compact license
CPR certification and valid driver's license required
Strong clinical judgment, documentation accuracy, and communication skills
Ability to work independently while collaborating across interdisciplinary teams
Strong communication, critical thinking, and documentation skills
Excellent communication, change management, and project execution skills are required
Demonstrates a strong commitment to delivering high-quality, person-centered care that reflects Arc Hospice & Palliative Care's mission, values, and standards of dignity, respect, compassion, and integrity

Preferred

Minimum one (1) year of hospice, palliative care, or home health experience preferred
Knowledge of hospice regulations and Medicare Conditions of Participation preferred
