Responsibilities
Perform comprehensive assessments for high-risk members
Collaborate with primary care providers to develop and implement individualized care plans
Identify and reassess appropriate, cost-effective levels of care
Coordinate care transitions across the continuum of care
Arrange non-medical support services, including housing and transportation
Partner with physicians, social workers, discharge planners, and interdisciplinary care teams
Engage specialty and community resources to optimize patient outcomes
Maintain accurate and timely documentation of clinical, functional, and financial outcomes
Identify opportunities for health promotion and illness prevention
Advocate for patients while ensuring privacy and confidentiality
Qualification
Required
Associate's or Bachelor's Degree in Nursing or related field
Active, unrestricted Registered Nurse (RN) license in the state of practice
Strong clinical judgment and ability to analyze complex medical information
Commitment to mandatory attendance during the first 90 days of employment
Preferred
Case Management certification preferred
